For over 20 years, children with chronic illnesses between 6 and 16 years old participate in a 3-day, 2-night annual adventure camp. It is a medically supervised camp where trained dedicated befrienders take care of these children while they join in the camp activities.

1. Registration is now closed.
2. Parents/guardians of all registered children to complete and sign the following hardcopy forms which includes:

ii. Medical Form (doctor to approve)
